VSD
This 61-year-old man has an acquired ventricular septal defect following a myocardial infarction. Surgical repair has been attempted but has failed. He has a pansystolic murmur consistent with a recurrent septal defect.
Tricuspid regurgitation
This 79-year-old lady with severe mitral regurgitation has developed pulmonary hypertension and, as a consequence, tricuspid regurgitation. She has a pan systolic murmur of mitral regurgitation at the apex and a pan systolic murmur of tricuspid regurgitation heard at the right sternal edge. The tricuspid regurgitation murmur is augmented by the patient holding her breath in inspiration.
